Just follow these 4 steps
Step 1
Fill out the form to the right providing us with your name, contact information, and reason for contacting us. Indicate if you would like a response by email or phone.
Step 2
You will be contacted as soon as possible to answer any questions, explain how we can help you, and complete the process of scheduling your first appointment. We try to return initial inquiries promptly, but it may take a few days to receive a response. Please include your phone numbers, as well as your email address. If your first appointment is by Skype, you will need to send a contact request prior to the appointment.
Step 4
Email a .jpg photo of the patient , as an attachment, to the doctor at her address below.
Step 3
Upon making your appointment, you will download, print, complete, and sign either the Adult or Child Medical History Form*. It is necessary to print the form, rather than filling out and submitting online, because we need your signature. We want that you either FAX or EMAIL the form back to us within 24 hours of downloading the form.
